Required Notice - Texas Health & Safety Code Section 181.105

1. Requesting Your Health Care Records

   • You have the right to request a copy of your health care records. To request records, submit a written request to UpTIC Consulting & Counseling, 7601 S. Western, Amarillo, TX 79110. Please include your full name, date of birth, contact information, and the records requested. You can also deliver your written request in person at your next appointment with your therapist.

2. Contact the Licensing / Oversight Agency

   • You have the right to file a consumer complaint (as outlined in Texas Health & Safety Code Section 181.103).
